














Keeping your information safe and secure is vital. If you want other people – like family members or support coordinators – to be able to access information and discuss your plan with us, you’ll need to complete a Client Consent to Obtain and Release Information form, so we know who to provide that access to.
You can fill the form out on your computer or print it and complete it in writing. Once you’ve filled it out, please email it to enquiries@myplanmanager.com.au or mail it to us c/o My Plan Manager at Level 9, 400 King William Street, Adelaide SA 5000.


Every NDIS provider – registered or not – is required to adhere to the NDIS Code of Conduct.
If you have concerns about any of your providers, there are people who can help. You can contact the NDIS Quality and Safeguards Commission or call My Plan Manager on 1800 958 018. You can also email us at enquiries@myplanmanager.com.au.



Accessing supports under the NDIS begins with connecting with the National Disability Insurance Agency or an NDIS partner – a team member of a community-based organisation funded by the Agency to help deliver the NDIS in some parts of Australia.
You can determine if you’re eligible to join the NDIS by using this checklist. If you meet the requirements, you’ll be supported to submit an application. Then, you’ll wait for the Agency’s decision.
